Facility Transfer Bond Release Application

In accordance with rule R649-9-2.4 et al General Waste Management, of the Utah Oil & Gas Conservation General Rules, every operator shall submit to the Division, an Annual Waste Management Plan (AWMP) by January 15 of each year to account for the proper disposition of produced water and other E&P Wastes. Please submit the completed forms to ReportingOG@Utah.gov

The Division of Oil, Gas and Mining has been working on implementing a new Salesforce-based application that will streamline the communications and form approval process with operators. The first application available through Salesforce is the Pollution Control Facility Tax Credit Application. Over the coming months more forms, permits and reports will be available, we will update you as these changes are made. Please email OG@utah.gov with a company name, contact person and their name and email to receive account setup instructions.
