The following examples of bid submittals are being
provided to assist contractors when
they are preparing their bid proposals.
These required bid submittals must be included in the
contractors bid proposal package that is delivered to the purchasing agent with
the State of Utah Division of Purchasing
1.02 SUBMITTALS
The
CONTRACTOR shall submit with the Bid Proposal a description of access improvements to be
performed at each site and shall not deviate from this plan without the written
approval of the OWNER. For sites not
easily accessible, the CONTRACTOR may choose to gain equipment access, do hand
work, or access with a helicopter.
Access will be on designated roadways identified in the construction
documents and on the maps. Small
boulders and debris may need to be moved from the access routs. Excessively rough areas of the road will be
leveled out. No culverts are needed.
1.02 SUBMITTALS
A. CONTRACTOR shall submit with the Bid Proposal demolition and
removal procedures and schedule for approval.
This should indicate if blasting is to be used in structure demolition.
No blasting will be used
for demolition. Structures will be
demolished using a track hoe and D9 Cat.
Demolished materials will be transported to a licensed landfill.
Section 0250: Mine Closures
1.02 SUBMITTALS
CONTRACTOR
shall submit with the Bid Proposal
a list of equipment to be used to complete this section of the WORK.
Typical mason tools will
be used for walls. A portable welder
and generator will be used for bat gates and shaft grates. Trackhoe excavator will be used for
backfilling shafts and adits.
B. CONTRACTOR shall submit with the Bid Proposal the proposed
construction procedures in writing.
Two hand crews will begin
on hand backfills, bat gates and block walls.
Trackhoe excavator will prepare/improve access route. When access routes are complete trackhoe
will begin structure demolition. After
demo trackhoe will begin machine backfills.

A. Concrete
CONTRACTOR
shall submit with the Bid Proposal
laboratory reports indicating that the supplier's concrete ingredients meet
requirements specified.

CONTRACTOR
shall submit with the Bid Proposal
the proposed construction procedures, including a description of the form
materials to be used and the foam application equipment or method.
Forms will be constructed
and placed in openings as directed by owner.
Foam will be prepared in buckets and poured into place. No application equipment will be used. Form materials will include chicken wire,
cardboard, plastic sheeting and paper.
1.02 SUBMITTALS
A. CONTRACTOR shall submit with the Bid Proposal a
list of equipment to be used to complete this section of the WORK.
One D9 Cat, and 399 Track
hoe excavator with a 5 cyd bucket.
B. CONTRACTOR shall submit with the Bid Proposal locations of borrow
areas for fill and topsoil for the approval of the OWNER when such areas are
not shown on the Drawings.
Areas for borrow and topsoil will be those identified in the construction specifications. No other borrow areas will be needed.
Section 0275: Material
Transport
1.02 SUBMITTALS
A. The CONTRACTOR shall submit with the Bid Proposal a list of equipment
to be used to complete this section of the WORK.
Two Volvo dump trucks
with pups will be used to haul materials.

CONTRACTOR
shall submit with the Bid Proposal
the schedule and plan for implementing drainage control measures and for
utilizing riprap, gabion mattress, or a combination of systems in accordance
with these Specifications and Drawings.
If a combination of systems is planned, specific locations for each item
shall be identified. Also included
shall be specifications of riprap or gabion mattress fill material if supplied
as import, and specifications for gabion mattress to be used.
Riprap will be obtained
from borrow area identified in the specifications and stockpiled next to the
stream channel. No gabion mattresses
will be used. Silt fence will be installed
prior to any earthwork.
B. The CONTRACTOR shall submit with the Bid Proposal a description of and
location of stream protection measures to be used at each site.
Three hundred yards of
silt fence will be placed as stream protection. Straw bales will be placed in existing drainage ways.
C. CONTRACTOR shall submit with the Bid Proposal the
location of borrow areas intended for riprap unless otherwise specified in
Section 0300: Specific Site
Requirements, or the Drawings.
All riprap will be
obtained from locations identified in the construction specifications.
D. CONTRACTOR shall submit with the Bid Proposal the
specifications for materials and installation if either culverts or bridges are
proposed for use.
Areas needing culverts
will be excavated and prepared with gravel bedding, culverts will be placed and
backfilled evenly.
1.02 SUBMITTALS
A. CONTRACTOR shall submit with the Bid Proposal the location of sources
of rock material for rock streambarbs and vortex rock weirs unless otherwise
specified in Section 0300: Specific
Site Requirements, or the Drawings.
Rock material will be provided from the borrow locations identified in the specifications. Other locations will be approved prior to collection.
B. CONTRACTOR shall submit with the Bid Proposal the location of sources
of live or dead plant materials intended for revetments and streambarbs unless
otherwise specified in Section 0300:
Specific Site Requirements, or the Drawings.
Materials for revetments
will be secured with the necessary permit from adjacent BLM lands.
1.02 SUBMITTALS
CONTRACTOR
shall submit with the Bid Proposal the names of one seed supplier, and alternate, to
be used for the seed mixtures required by these specifications.

B. CONTRACTOR shall submit with the Bid Proposal a written
description indicating equipment to be used to perform the work required in
this section.
Seeding will be done by
hand broadcasting or seeding will be done using whirlygig spreaders

A. CONTRACTOR shall submit with the Bid Proposal the names of one or
more suppliers to be used for fencing required by these specifications.

B. CONTRACTOR shall submit with the Bid Proposal a written
description indicating equipment to be used to perform the work required in
this section.
A 4-wheeler with a back
mounted 4” auger drill will be used to establish post holes. Standard fencing tools will be used to
stretch and attach wire to posts.
